We believe that the path to sustainable change for the health of our communities lies in multi-sector, collaborative, interdisciplinary efforts that address the roots of health disparities, not the symptoms.

The Blue Hills Community Health Alliance, a hosted program of Bay State Community Services (BSCS), works to improve the overall health of local residents by facilitating multi-sector collaboration to address the region’s most pressing public health needs.

The Alliance serves the 13 communities of Braintree, Canton, Cohasset, Hingham, Hull, Milton, Norwell, Norwood, Quincy, Randolph, Scituate, Sharon and Weymouth.

One of the factors that often frustrates both patients and care providers is a disjointed system — one in which a primary care doctor handles some aspects of a person’s well-being, while another provider, such as a mental health professional, takes care of another aspect of that person’s wellness.
